I do hereby irrevocably consent and agree that 'Suzi Gardner Photography', it's employees, or agents have the right to take photographs, videotape, or digital recordings of me, and/or members of the arranged photo shoot parties who I represent, including but not limited to family members, friends, associates etc, on the date below;

I do hereby release to 'Suzi Gardner Photography' it's agents and employees all rights to exhibit this work in print and electronic form publicly or privately and to exploit for the purposes of marketing, public display, portfolio display, commercial or non commercial media publication.

I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used.

I further consent that my name/identity may be revealed therein or by descriptive text or commentary.

I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or payback.

I also understand that 'Suzi Gardner Photography' is not responsible for any expense or liability incurred as a result of my participation in the recording, including medical expenses due to any sickness or injury as a result.

I represent that I am at least 18 years of age, have read and understood the foregoing statement and am competent to execute this agreement.

I understand that I may breach this contract however I will be liable to pay in full the Suzi Gardner Photography standard session fee plus all costs incurred in house or third party on behalf of Suzi Gardner photography in the shooting and processing of images and also I will be due to pay for any images I have received or used in any form in full immediately.
